    System Hospital has started to provide services in the health sector with its high-tech infrastructure, quality and reliable understanding that prioritizes patient safety and privacy.

    Starting our quality journey during the construction phase of our hospital, all facilities, equipment and medical processes were established according to the Ministry of Health service quality standards and International JCI quality standards.

    In line with the mission and vision of our hospital, it is our most important duty and responsibility to undertake the highest level of patient satisfaction and give importance to patient privacy.

    Our goal as a corporation is to continue to grow with satisfied patients and happy employees by providing up-to-date diagnosis, treatment and care services with evidence-based current medical practices and reducing risks for both our patients and employees.

 

ENVIRONMENTAL POLICY

System Hospital is performed its duty by respectful to the environment, sensitive and aware of responsibility when it works with quality service mentality in line with its mission and vision,

Complies with all local and national environmental laws and regulations and fulfills all environmental requirements within the JCI standard.

A waste management plan is established to identify the wastes generated as a result of the priority activity and to remove the wastes in a way that does not threaten the health of patients and employees.

In line with this plan, waste is tried to be minimized and recyclables are separated.

The non-recyclable wastes are delivered to the relevant institutions for disposal in accordance with the laws and regulations.

In this direction; in the hospital that is planned to be environment friendly, clean and wastewater is separated and used in garden irrigation thanks to the water treatment system.

In order to create environmental responsibility in our company, all our employees are informed and subjected to training according to their individual responsibilities.

Taking into consideration the safety of our patients and relatives, we want to inform you about the “Personal Data Protection Law” which is regulated with regard to personal data in order to protect the privacy, fundamental rights and freedoms.

 To inform you in the most transparent way about the ways of receiving personal data, the purpose of processing, legal reasons and rights in line with the satisfaction of patients and their relatives.

We take all kinds of technical and administrative measures to ensure the necessary security measures for the protection of any personal data we receive from our patients.

This information is intended to inform our patients about the collection, processing, protection and sharing of all personal data within the scope of KVKK.

According to the Personal Data Protection Act No. 6698 (KVKK), your personal data will be processed by the hospital as the data responsible within the scope of this law.

 

General information about Personal Data Law

Law No. 6698 on the Protection of Personal Data (hereinafter referred to as KVKK) was adopted on 24 March 2016 and published in the Official Gazette dated 7 April 2016 and numbered 29677. Some of the KVKK was published on that day, others published on 7 October 2016.

 

Information acting in the capacity of the data officer

In accordance with KVKK No. 6698 and as a Data Officer, your personal data is within the framework of legal regulations; will be recorded, stored, updated, explained/transferred to third parties where permitted by applicable legislation, classified and processed in the manner specified in KVKK.

 

How your personal data can be processed

Pursuant to KVKK no. 6698, your personal data that you share with our hospital may be obtained completely, partially or automatically, or as part of any data recording system nonautomatically, recorded, stored, modified, rearranged and It may be processed. Any transaction performed on the data within the scope of KVKK is considered as “processing of personal data”.

Identity Information: Name, Surname, T.R. ID Number, Place of Birth, Date of Birth, Gender, Age

Contact Information: Address, Phone Number, E-mail Address and other contact information

Payment and Billing Information

SSI and Private Health Insurance Information

Health Information: All kinds of health data such as Medical Examination Information, Health Reports, laboratory results, Pathology Results, Visualization Results and Reports, Surgery and Medical Intervention Information, Surgery and Medical Intervention Reports, Patient Monitoring Information, Drug and Application Information, Diet Information, Surgery Images.

Health File Documents: Consent forms, undertakings and other documents obtained from patients or their relatives in order to perform health service and provide the legal infrastructure of the service provided

CCTV Images: Security camera images in and around the hospital

Telephone Voice Recordings: Voice recordings of the calls you have made with our hospital call center and other extension lines.

Internet Traffic Records: Traffic information recorded in case you use wired-wireless networks of our hospital

License Plate Information: License plate information for vehicles that benefit from our parking service

Personal and Other Data of Commercial Relations

Conditions for the processing of your personal data

(1) Personal data cannot be processed without the express consent of the person concerned.

(2) If one of the following conditions exists, it is possible to process personal data without the express consent of the person concerned:

a) To be explicitly foreseen by law.

b) It is compulsory for the protection of the life or body integrity of the person or someone else who is unable to disclose his or her consent due to the impossibility of the person or whose consent is not granted legal validity.

c) The processing of personal data of the parties to the contract, provided that it is directly related to the establishment or performance of a contract.

ç) Publication by the person concerned.

d) Data processing is mandatory for the establishment, use or protection of a right.

e) It is compulsory to process data for the legitimate interests of the data officer, provided that they do not harm the fundamental rights and freedoms of the person concerned.

Conditions for the processing of personal data

ARTICLE 6 - (1) Data on race, ethnic origin, political opinion, philosophical belief, religion, sect or other beliefs, dress, association, foundation or union membership, health, sexual life, criminal conviction and security measures biometric and genetic data are sensitive personal datas.

(2) The processing of sensitive personal data without the express consent of the person concerned is prohibited.

(3) Personal data other than health and sexual life listed in the first paragraph may be processed without the express consent of the person concerned in cases provided for by law. Personal data related to health and sexual life, but only for the protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing, confidentiality of the persons under the obligation of secrecy or authorized institutions and organizations without seeking the express consent of processed.

(4) In the processing of sensitive personal data, it is also necessary to take adequate measures determined by the Board.

Deletion, destruction or anonymization of personal data

ARTICLE 7 - (1) Although processed in accordance with the provisions of this Law and other relevant laws, personal data are deleted, destroyed or made anonymous in case the reasons that require processing are eliminated and ex officio and upon the request of the person concerned.

The anonymization of personal data means that the personal data cannot be associated with any identifiable or identifiable natural person, even if it is matched with other data.

In order to make personal data anonymous; personal data cannot be associated with an identifiable or identifiable natural person, even through the use of appropriate techniques for the recording medium and field of activity, such as the return of data by the data officer, recipient, or groups of recipients, and the matching of data with other data.

Your personal data are collected and processed as follows:

  • By applying the patients or their relatives to the hospital and taking them verbally, in writing and electronically during the patient registration;
  • During the provision of health services, oral, written or electronically taken or created by the new method;
  • During the collection and billing, written or electronically through the oral, written or electronically received by the method;
  • By the CCTV system, telephone exchange and call center through the method of receiving electronically;
  • During the use of the Internet service of the hospital by means of electronic reception;
  • Web sites, blogs, electronic surveys, social media channels and e-mail with the method of receiving electronically;
  • It can be received via the marketing unit and call center orally, in writing or electronically;
  • By means of oral, written or electronic means taken from people who share their personal data by means of business cards, CVs and other means that are shared for the purpose of establishing commercial relations with our institution and applying for a job;
  • The data provided by the public databases are based on the profiles and data available for sharing through social media platforms.

 

Purpose of Use of Your Personal Data

Your personal data is collected to enable the organization to fulfill its obligations arising from the applicable legislation and statutory and contractual agreements, to provide health and support services, and to conduct business operations.

It is used:

  • In order to fulfill the legislation and legal obligations:
    • Basic Law of Health Services No. 3359,
    • Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations,
    • Private Hospitals Regulation,
    • Fulfilling our legal obligations in the Regulation on the Processing and Protection of Personal Health Data and other relevant regulations;
  • For the purpose of protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing;
  • In order to regulate the processes related to the institution and to increase the service quality;
  • In order to verify your rights under the health contracts;
  • In order to make corporate billing;
  • In order to respond to the requests of other public institutions and organizations, especially the Ministry of Health.

 

Your personal data obtained before the KVKK enters into force

Prior to the effective date of KVKK, 7 April 2016, your personal data obtained in accordance with the law, such as membership, purchase of products/services, electronic message permission and other forms, are processed and maintained in accordance with the terms and conditions set forth in this document.

 

Storage and protection of personal data

Your personal data will be kept confidential by our institution in physical and digital environments in accordance with Article 12 of KVKK; will not be shared with third parties in any way other than legal and legislative obligations and regulations specified in this document.

Our company is obliged to prevent the unlawful processing of personal data in accordance with Article 12 of KVKK, to prevent unauthorized access, and to take software and physical security measures such as access management.

If it is learned that others obtain the personal data illegally, the situation will be reported to the Personal Data Protection Board immediately in writing in accordance with the legal regulations.

 

Keeping personal data up-to-date and accurate

Pursuant to Article 4 of KVKK, our Company has an obligation to keep your personal data accurate and up-to-date. Within this scope, in order to fulfill its obligations arising from the legislation in force, our customers are required to share true and current real data.

 

Rights of personal data holder in accordance with KVKK No. 6698ı

Article 11 of the KVKK no. 6698 entered into force on 07 October 2016 and in accordance with the related article, the Personal Data Holder may apply to our institution and benefit from the following rights regarding his personal data:

  • Learning whether personal data is processed,
  • Requesting information if personal data is processed,
  • Learning the purpose of processing personal data and whether they are used in accordance
  • with their purpose,
  • Knowing the third parties to whom personal data is transferred at home or abroad,
  • Requesting correction of personal data in case of incomplete or incorrect processing,
  • Requesting the deletion or destruction of personal data in accordance with the conditions provided for in Article 7 of the KVKK,
  • In case of correcting, deleting, destroying personal data, requesting that these transactions be notified to the third parties to whom the personal data are transferred,
  • Objecting to the occurrence of a result against the person himself by analyzing the processed data exclusively through automated systems,
  • In the event that the personal data is damaged due to the unlawful processing of the personal data, it has the right to demand the loss of the damage.

It is important that the personal data we hold about you are accurate and up to date. Therefore, please let us know when a change in your personal data occurs. In accordance with the Personal Data Protection Law No. 6698 and related legislation;

TARGET OF QUALITY

    To define the roles and responsibilities of all staff in the hospital from senior management to department employees, to establish a quality management structure, to plan, conduct and coordinate quality improvement activities and to ensure continuous improvement of quality. Our aims are to maximize patient safety, employee safety and facility safety by taking the opinions of patients, patient relatives and employees.

DUTY, AUTHORITY AND RESPONSIBILITIES OF CORPORATION’S QUALITY MANAGEMENT UNIT

- Ensures the coordination of the activities carried out within the framework of the SKS,
- - Manages self-assessments,
- It determines the performance and quality targets of the corporation by evaluating the corporation’s current situation in terms of efficiency, effectiveness, service quality of patient and employee safety. Plans, implements, controls the necessary studies in order to realize these targets and ensures that improvement works are carried out according to new situations,
- Manages the studies for measuring patient and employee satisfaction (such as survey applications, evaluation of survey results, improvement studies for survey results, receiving patient and employee feedback),
- Plans, provides and manages all necessary resources to improve patient care quality and patient and employee safety,
- Plans, provides and manages all necessary resources in order to improve the performance and quality of our hospital,
- Provides the management of documents within the framework of SKS,
- Determines the existing problems in the organization, prioritizes them and carries out process improvement works and manages the processes related to the Security Reporting System,
- Provides the establishment of an incentive mechanism to ensure the voluntary participation of the employees of the Agency in this activity,
- Provides communication and cooperation with Kayseri Health Directorate Quality Unit and other hospitals and public institutions regarding performance and quality management activities,
- Manages the risk management processes with the OHS Committee,
- Manages processes for quality indicators,
- Participates in the committees determined within the framework of the SKS and manages the meetings,
- The quality unit meets with the responsible persons every four months and determines the level they come from in the activity plans and makes new decisions according to the results and applies with the approval of the hospital manager.
